Purpose of the research study: The purpose of the study is to understand how adults feel about the walking they do each day. The questions about your trip here today will help us understand attitudes towards walking.

What you will do in the study: You will answer some general questions about your walking habits and route to this location. Anything you report will be kept confidential; you don't have to answer any questions that you do not want; and you may stop at any time without consequence.

Time required: The survey will require about 5 to 10 minutes of your time.

Risks: There are no anticipated risks.

Benefits: There are no direct benefits to you for participating in this research study. The study may help us understand how adults feel about the walking they do each day.

Confidentiality: Your responses will be confidential and we do not collect identifying information such as your name, email address or IP address. All data is stored in a password protected electronic format. To help protect your confidentiality, the surveys will not contain information that will personally identify you.

Voluntary participation: Your participation in the study is completely voluntary.

Right to withdraw from the study: You have the right to withdraw from the study at any time without penalty.

How to withdraw from the study: If you want to withdraw from the study, simply stop taking it and close your web browser.

Payment: You will receive a $5 Starbucks card for participating in the study.
